What Is a Cybersecurity Risk Assessment?
A cybersecurity risk assessment is a structured process for identifying, analysing, and prioritising the security risks that exist across a system, product, or organisation. It looks at what assets are present, what threats are realistic, what vulnerabilities exist, and what the consequences of exploitation would be. The output is a prioritised picture of where risk actually sits. Not a theoretical inventory, but a working map of what needs attention and in what order.
The process varies depending on what is being assessed. A risk assessment for a web application looks different from one for an industrial control system. The methodology and the tools differ. The threat models differ. The consequences of a finding differ. What does not differ is the intent: find out what could go wrong, understand the likelihood and impact of each scenario, and give the people responsible for the system something actionable to work from.

The output is not a comprehensive list of every conceivable attack. It is a prioritised set of realistic attack scenarios that the rest of the assessment tests against.
Vulnerability identification finds the weaknesses in the system that the threat scenarios could exploit. This combines automated analysis, configuration review, architecture assessment, and where appropriate, technical testing. The goal is not to find every theoretical imperfection. It is to find the vulnerabilities that, given the threat model, represent real exposure.
Risk analysis scores each finding by combining the likelihood of exploitation with the potential impact. The output is a prioritised risk register that gives the people responsible for the system a clear, ranked picture of what needs to be addressed and in what order. Risk Assessment for OT and ICS Environments
Operational technology and industrial control system environments present challenges in risk assessment that do not arise in standard IT or software contexts. Understanding those challenges is necessary to conduct a risk assessment that accurately reflects the exposure these systems carry.
The first challenge is the legacy problem. OT environments regularly include equipment with decades-long operational lifespans, running protocols designed before cybersecurity was a design consideration. Many of these systems cannot be patched, cannot be replaced without significant operational disruption, and are not represented accurately in current asset inventories. Where Risk Assessment Fits in the Development Lifecycle
Risk assessment is most effective when it is treated as an ongoing activity integrated throughout the development lifecycle, rather than a single exercise conducted before a product ships or a system goes live. The earlier a risk is identified, the cheaper it is to address. A vulnerability that reflects an architectural decision costs far more to remediate after deployment than one caught during design.
At the design stage, risk assessment informs architecture decisions. Threat modelling during design identifies the attack surfaces a proposed architecture creates, the trust boundaries it needs to enforce, and the failure modes it needs to handle safely. These questions are cheapest to answer before implementation begins and most expensive to revisit after a product is in the field.

What Good Risk Assessment Output Looks Like
The value of a risk assessment depends almost entirely on what is done with the output. An assessment that finds significant vulnerabilities but produces a report that cannot drive remediation is not a successful assessment. Understanding what good output looks like helps teams commission assessments that produce results they can act on.
Each finding needs a precise description of what was found, where it was found, and how it was identified. Vague descriptions of vulnerability classes are not sufficient. The finding needs to be specific enough that the team responsible for the system can locate the vulnerability, understand its mechanism, and verify that a fix has been effective.
The risk scoring attached to each finding needs to reflect exploitability and impact in the specific deployment context, not just the generic severity of the vulnerability class. A critical-severity vulnerability that is only reachable from inside a tightly controlled network perimeter presents a different level of actual risk than the same vulnerability exposed on a public interface. The scoring needs to reflect that distinction, or it will not support meaningful prioritisation.
Recommendations need to be actionable within the operational constraints of the system being assessed. A recommendation to replace a legacy device that has been in service for twenty years and is integral to a production process is not actionable in the short term. The output needs to include interim mitigations that reduce risk while longer-term remediation is planned, not just the theoretically correct final state.
For compliance use cases, the output needs to map findings to specific framework requirements. IEC 62443-4-1 Practice 6 requires vulnerability testing evidence with documented scope, methodology, and traceability to specific requirements. An assessment report that produces findings without connecting them to the framework requirements the assessment was commissioned to address does not satisfy that obligation.

A penetration test actively attempts to exploit vulnerabilities. A risk assessment identifies and prioritises them.
